Las Vegas, NV —A tandem skydive with an instructor and a 75-year-old grandmother ended in tragedy when two parachutes did not open and both people died. Claudette Porter of North Las Vegas received the skydiving adventure as a gift for her birthday from her granddaughter. The instructor, James Fonnesbeck, was a veteran skydiver who had over 11,000 jumps.
